Intake Information - Open Arms Clients

Confident Horizons is proud to be part of the Open Arms Outreach Counselling program, and we look forward to working with you towards your therapy goals.

Confident Horizons is committed to ensuring your privacy and will maintain the highest level of confidentiality. Information disclosed during therapy sessions will only be shared if permission is granted by the client, or in circumstances related to duty of care and legal obligation. At Confident Horizons, we use a platform called Heidi to record clinical notes during sessions. This allows us to focus entirely on you during your visit while still allowing a comprehensive and precise record of your care. All information is de-identified and encrypted and Heidi complies Australian Privacy Principles and the Privacy Act. You can find more information about Heidi on this factsheet or via their website. Please speak to your therapist if you have any concerns.

Prior to your first appointment, Open Arms requests that the following screening tools be completed:

  1. Depression and Anxiety Symptom Scale (DASS21)

  2. Alcohol Use Disorders Identification Test (AUDIT)

    1. Please note, the AUDIT screener is not an electronic form, but you can ‘add text’ to the PDF to enter the required information

The DASS21, AUDIT, and Privacy Consent Notice can be completed electronically and emailed directly to hello@confidenthorizons.com.au. Alternatively, please arrive 15 minutes prior to your appointment time to complete this paperwork. If you would prefer to not complete the screening tools, you can discuss this with your therapist.
